fa مقایسه اثرات درمانی لیزر باطول موج (nm585 (PDL باکرایوتراپی در درمان زگیل عمومى General كاربردي Applicable <p style="text-align: justify;"><span style="line-height:2;"><span style="font-family:Tahoma;"><span style="font-size:14px;"><strong>مقدمه</strong>: زگیل پرولیفراسیون خوش‌خیم یاتومور پوستی مخاطی در اثرگونه های مختلف HPV است که میتواند درنواحی مختلف پوست ومخاط بیماری ایجاد کند.این بیماری بین کودکان ونوجوانان بسیار شایع است که برای درمان آن از روشهای متفاوتی استفاده میشود. با توجه به عوارض ناشی از کرایوتراپی وهمچنین میزان اثربخشی متغیر درمان بالیزر پالسی رنگی در مطالعات مختلف صورت گرفته تصمیم به انجام این مطالعه گرفته شد. هدف مقایسه تاثیروعوارض pulsed dye laser ( PDL ) باطول موج nm 585و کرایوتراپی در درمان زگیل می‌باشد.</span></span></span></p> <p style="text-align: justify;"><span style="line-height:2;"><span style="font-family:Tahoma;"><span style="font-size:14px;"><strong>روش بررسی: </strong>درطی یک کارآزمایی بالینی آینده نگر شاهددار که در سال 1389در بیمارستان افضلی پور کرمان بر روی 100ضایعه زگیل در 20 بیمارانجام شد. بیماران بطور تصادفی به دو گروه درمان با لیزر رنگی پالسی pulsed dye laser ( PDL ) باطول موج nm 585 (50ضایعه در 10بیمار)ودرمان توسط کرایوتراپی(50ضایعه در 10بیمار)تقسیم ودرمان تا پاک شدن زگیل هر سه هفته یکبارتاپنج نوبت تکرار شد.بیماران در هر نوبت معاینه واز نظر پاسخ درمانی واحتمالا پیدایش عوارض بررسی می‌شدند.پس از کامل شدن دوره درمان بیماران هر 4هفته یکبار تا دوماه از نظر پیدایش عوارض یا عود ضایعه تحت پیگیری قرار گرفتند.</span></span></span></p> <p style="text-align: justify;"><span style="line-height:2;"><span style="font-family:Tahoma;"><span style="font-size:14px;"><strong>یافته ها: </strong>میزان بهبودی روش کرایوتراپی 96 درصد و روش لیزر درمانی با لیزر رنگی پالسی 82 درصدبود. در نتیجه میتوان گفت که از نظر بهبودی روش کرایوتراپی بهتر از روش لیزردرمانی است ولی میزان بهبودی در دو گروه از لحاظ آماری با هم تفاوت معناداری نداشتند ( P.V=0.056 ).</span></span></span></p> <div style="text-align: justify;"><span style="line-height:2;"><span style="font-family:Tahoma;"><span style="font-size:14px;"><strong>نتیجه گیری: </strong>با توجه به کمتر بودن میزان عوارض ایجاد شده در گروه لیزر تراپی بنظر میرسد که میتوان این روش را بعنوان اولین روش درمانی بکار برد گر چه برای رسیدن به چنین تصمیمی نیاز به مطالعات گسترده تری داریم.</span></span></span></div> <span style="font-size:10pt"><span style="font-family:Arial,sans-serif"><span style="font-weight:bold"><span style="font-size:11.0pt"><span style="font-family:&quot;Times New Roman&quot;,serif">Background:</span></span><span style="font-size:11.0pt"><span style="font-family:&quot;Times New Roman&quot;,serif"><span style="font-weight:normal"> Common wart is a benign proliferation or tumor of mucocutaneous surfaces caused by different types of human papilloma virus (HPV). It is very common in children and young adults.</span></span></span></span></span></span><br> <span style="font-size:10pt"><span style="font-family:Arial,sans-serif"><span style="font-weight:bold"><span style="font-size:11.0pt"><span style="font-family:&quot;Times New Roman&quot;,serif"><span style="font-weight:normal">Objective: The aim of this study is to compare therapeutic and side effects of Cryotherapy versus Pulsed Dye Laser (PDL) in the treatment of common warts.</span></span></span></span></span></span><br> <span style="font-size:10pt"><span style="font-family:Arial,sans-serif"><span style="font-weight:bold"><span style="font-size:11.0pt"><span style="font-family:&quot;Times New Roman&quot;,serif">Material and Methods:</span></span><span style="font-size:11.0pt"><span style="font-family:&quot;Times New Roman&quot;,serif"><span style="font-weight:normal"> In this prospective controlled randomized clinical trial (RCT), 20 patients with 100 wart lesions were enrolled. The patients were randomly divided into two groups each consisting of 10 patients (with 50 lesions). The treatment protocol for the first group consisted of laser therapy with PDL (wavelength of 585 nm), and the second group treated with Cryotherapy. Patients in both groups were treated every 3 weeks up to 5 times till complete clearance of the lesions achieved. All of the patients were examined in every session of treatments for the clinical response or probable side effects. After completing the proposed course of treatments, patients were examined every 4 weeks for 2 months in order to detect recurrence or probable side effects.</span></span></span></span></span></span><br> <span style="font-size:10pt"><span style="font-family:Arial,sans-serif"><span style="font-weight:bold"><span style="font-size:11.0pt"><span style="font-family:&quot;Times New Roman&quot;,serif">Results:</span></span><span style="font-size:11.0pt"><span style="font-family:&quot;Times New Roman&quot;,serif"><span style="font-weight:normal"> The clinical response for the first group (treated with PDL) and second group (treated with Cryotherapy) were 46.1% and 53.9% respectively.
